FIRST VISIT

Welcome and mahalo for selecting us for your healthcare needs. We would like your first visit to be a warm and pleasant one. We hope the information below is helpful to you.

The patient experience within the office is smoother when we see patients by appointment. We encourage all patients to call for an appointment. Our schedule has been planned in a way to try and prevent long waits and provide a good balance of sick and well appointments. Emergencies do occur and there are occasional delays in appointments. Patients with appointments are seen before walk-ins. We have daily same day appointments available to see you or your child should an illness arise.

After you have checked in with the front desk, please have a seat in our comfortable waiting area. Your nurse will direct you to the examination room and ask questions about your reason for the visit. After the doctor has visited with you, you will be provided a checkout slip to be presented to the receptionist. They will give you a future appointment, if needed. All payments for services rendered are due at time of service.
Please remember to bring any medications you, or your child may be taking and your child's immunization records when coming for physicals and shots.

If you are unable to keep a scheduled appointment or think you will be late, please notify the receptionist as soon as possible for rescheduling. We kindly request that all cancellations be made within 24 hours of the scheduled appointment to allow us to offer the appointment to another patient.

Below are our office documents we will use for your first visit. They are available to download. Please print them and complete, and bring along to your first office visit.
